They do this by evaluating the psychosocial and mental health issues of patients to ensure transition planning, crisis management, and planning/coordination of both internal and external services. Provides key social work services in a variety of pediatric settings across the health care continuum. Key services include (but are not limited to): psychosocial assessments, crisis management, planning and coordination of internal and external services, resource connection, transition/discharge planning, supportive counseling, psychosocial education, post-discharge follow-up, and in limited settings, concurrent utilization review.
Masters degree in social work from an accredited school of social work.
Minimum one year post-Masters experience in one or more of the following areas required: Healthcare, discharge planning, case management or coordination of home based services, or has previously completed a Legacy Social Work internship. Pediatric medical experience strongly preferred.
Broad knowledge of the principles of social work and the ability to apply them
Demonstrates strong organizational skills, including an ability to effectively triage patient and family needs
Practices with strengths based practice focus
Engages in culturally competent practice
Demonstrates excellent verbal and written communication skills for effective and appropriate interactions with patients, families, physicians and staff
Effectively engages in discharge/transition planning
Demonstrates ability to locate, utilize and advocate for patient/family access to hospital/ community resources
Exhibits knowledge of legal/regulatory issues and an ability to be a resource to all team members regarding such issues
