The Family Navigator (FN) is a Social Worker with comprehensive knowledge of trauma treatment modalities, crisis intervention, and case management.  The FN demonstrates critical thinking and utilizes social work ethics to assess and meet the needs of patients and their families/caregivers by providing care coordination throughout the care continuum. 

FN assists in improving the clinic care experience and preparedness for treatment by orienting and educating patients, families, and caregivers to the healthcare system and multidisciplinary team member roles and resources.  FN will assess and address patients’ barriers to care, partner with patients/families to address their concerns, issues and needs, and facilitate appropriate referrals.  FN will serve as the central point of contact to the patient, facilitating collaboration among all team members and ensuring an individualized approach throughout the length of the patient’s needs. FN will demonstrate commitment to quality patient care, implement creative and innovative ways to meet diverse patient needs, and demonstrate ability to prioritize utilizing organizational skills and clinical judgement. FN will promote a consistent, holistic plan of care to enhance the quality of life of patients, families and caregivers through assessment, teaching, communication, support, and the practice of family-centered care. FN will work to influence both positive patient outcomes and system outcomes through improved interdisciplinary communication, patient retention, and community partner collaboration.

The Family Navigator in the PSB Clinical Coordinator position will oversee a multi-level response to PSB for children in Multnomah County.  The PSB Clinical Coordinator will lead the Multnomah County PSB committee, manage CARES Northwest’s clinical response to PSB referrals, and strengthen Multnomah County’s systemic response to PSB through outreach, engagement and collaboration in the community. 

Education: 

Master’s degree in mental health field, including Social Work, Psychology, Counseling, or Marriage and Family Counseling.

Experience:  Minimum one-year post-MSW experience in one or more of the following areas required: Healthcare, family service, child maltreatment, case management, or coordination of home-based services.  Bilingual in English and Spanish is preferred.

Skill Requirements:

Broad knowledge of the principles of Social Work and the ability to apply them.

Demonstrates strong organizational skills, including an ability to effectively triage patient and family needs.

Practices with strengths-based practice focus.

Engages in culturally competent practice.

Demonstrates excellent verbal and written communication skills for effective and appropriate interactions with patients, families, staff, and community partners.

Effectively engages in discharge/transition planning.

Demonstrates ability to locate, utilize and advocate for patient/family access to community resources.

Exhibits knowledge of legal/regulatory issues and an ability to be a resource to all team members regarding such issues

Licensed Clinical Social Worker (LCSW) or Clinical Social Work Associate (CSWA, working toward LCSW), Licensed Master’s of Counseling (LPC) or MA/MFT working toward LPC (LPC-Intern) or LMFT required within six months of hire date for employees who perform this job in the state of Oregon.
