• Manage patients in the ambulatory care setting through a collaborative, team-based model involving providers, clinic staff, resource specialists, and clinical resource coordinators.

• Provide disease state management services to support chronic condition care and therapeutic outcomes.

• Deliver patient education to improve understanding, adherence, and self-management of prescribed therapies.

• Optimize drug therapy regimens in partnership with providers, ensuring safe, effective, and cost-conscious medication use.

• Contribute to the design and implementation of clinical initiatives and care models in collaboration with clinic teams.

Education: 

• Graduate of School of Pharmacy. 
• Advanced degree (Doctor of Pharmacy preferred) with residency training or equivalent work experience required.

Experience: 

• Experience in ambulatory setting with disease state management preferred.

• Current applicable state Registered Pharmacist License (RPh) required.
