Overview

As a Behavioral Health Clinician (BHC), you will work in a consultative and collaborative role as an integral member of the primary care team, providing behavioral health interventions that support the overall health and well-being of patients. In this role, you will help address a broad spectrum of behavioral health needs within the primary care population by identifying concerns early and providing problem-focused, evidence-based interventions that promote wellness and relieve symptoms. You will assess, manage, and triage psychological and social concerns related to both physical and mental health conditions, partnering with patients and the care team to deliver integrated, patient-centered care.

Responsibilities

  • This position requires advanced level skill in delivering evidence-based interventions targeted to improve overall health for patients with a variety of emotional and behavioral problems.
  • The work includes the BHC meeting with patients individually for an assessment and then providing recommendations to the primary care team to enhance the patient's care plan. 
  • When indicated, the BHC may also work with patients for several subsequent sessions to provide on-going skills training and coaching to improve health and functioning.
  • The BHC also supports helping patients, who require services not provided in the LMG clinic, to make a smooth transition to care with community partners.

Qualifications

Education:

  • Master of Social Work, Master in Professional Counseling, Master of Family Therapy degree from a nationally accredited institution, fulfilling the requirements for licensure in the state of Oregon and/or Washington.

Experience:

  • Minimum of two years of clinical experience or healthcare related field.

Skills:

  • Ability to diagnose common mental health problems. Able to conduct solution focused interventions. Modalities including ACT, CBT, CBT-I, and psychodynamic psychotherapy are all applicable.
  • Recognizes and advocates for patients regarding behavioral health strategies/interventions.
  • Promotes health behavior change through evaluation, brief intervention and education.
  • Works with multidisciplinary team to help support the patient’s health. Coordinates with LMG providers and community partners to refer for appropriate mental health therapy.
  • Demonstrates competencies in assessment of emotional, cognitive and behavioral disorders in child, adolescent, adult and/or geriatric patients (depending upon population served by specific position). 
  • Demonstrates competency in assessment of cognitive and psychological problems associated with, and/or exacerbating medical conditions.
  • Experience with working with an interdisciplinary team.
  • Skilled in case conceptualization and consultation.
  • Flexibility and willingness to work at the fast pace of primary care.

Licensure

  • Licensed in one of the following within applicable state:
    • Clinical Social Worker (LCSW)
    • Licensed Independent Clinical Social Worker (LICSW)
    • Licensed Professional Counselor (LPC)
    • Licensed Mental Health Counselor (LMHC)
    • Licensed Marriage and Family Therapist (LMFT)
